In my UMass Amherst account, it says that the laboratory section is worth 0 semester credits and then the main lecture section is worth 4 credits. But, when I try submitting this form with 0 credits for the lab section, it says "Cannot be blank if course has associated lab," so I just put 0.25 so that I can submit this form, but it's supposed to be 0 credits. My lab and main course share the same syllabus, so I just submitted that single syllabus. Also, on my UMass Amherst Account, it doesn't actually show me any information about grading or course name for the lab section. It just shows grades and course name for the main course (lecture): CICS 210 Data Structures. So, I just put "Data Structures Laboratory" as the name for the lab, but officially, I don't think UMass gave it a name. I don't know what a Course Document is or how it differs from a course syllabus, so I just submitted my syllabus. I believe that this course is most similar to Georgia Tech's CS 1332: Data Structures and Algorithms for Applications. They both have the exact same textbook, cover extremely similar content, and even though CICS 210 is called Data Structures, it also includes many algorithms, as seen in the syllabus. Also, I just want to specify that the lab section definitely does exist, it's just that on my UMass Account, it's labeled as 0 credits since CICS 210 (the class) is worth 4 credits, and they just labeled the lecture (main) section as 4 credits and the lab section as 0 credit so that they both added up to 4 credits. On the Transfer Equivalency website, it currently says "Approp. Dept. Must Evaluate"
